Humanin (HN) was found in 2001 by a functional screen of a cDNA library from an Alzheimer disease brain. The cDNA encoded a 24-residue peptide that blocked neuronal death caused by familial Alzheimer disease genes and by amyloid-beta, but not death caused by polyglutamine (Q79) or mutant superoxide dismutase 1 (PMID 11371646, PMID 11717357). The sequence is MAPRGFSCLLLLTSEIDLPVKRRA (UniProt Q8IVG9). It matches an open reading frame inside the mitochondrial 16S rRNA gene MT-RNR2. UniProt adds a caution: if the sequence were translated inside mitochondria with the mitochondrial genetic code, the product would lack the last three residues, and the active peptide could instead come from one of about 13 related nuclear loci (MTRNR2L1 to MTRNR2L13; PMID 19477263). A 2002 study detected HN-immunoreactive protein of about 3 kDa in mouse testis and colon and in an Alzheimer brain, which supports in vivo production (PMID 12009529). Reported molecular actions, from cell and animal work: HN binds BAX and stops its move from cytosol to mitochondria (PMID 12732850); HN also binds BID and BimEL (UniProt Q8IVG9); HN binds IGFBP-3 at its 18-residue heparin-binding domain (PMID 14561895); HN is a ligand for the G protein-coupled receptors FPRL1 (FPR2) and FPRL2 (FPR3), and the N-formylated form is far more potent at FPRL1 (EC50 0.012 nM against 3.5 nM; PMID 15465011); and HN binds a receptor complex made of CNTFR, WSX-1 (IL27RA) and gp130 (IL6ST), which UniProt links to STAT3 signalling (PMID 19386761). HN also improved insulin action in rodents when given into the brain or as an analog by vein (PMID 19623253), and extended life span in C. elegans when overexpressed (PMID 32575074). The S14G analog (HNG, serine 14 replaced by glycine) is about 1000-fold more potent than HN in neuroprotection assays (PMID 16945331). Most animal intervention studies use HNG, not native HN. HNG is listed here as an alias because the same literature covers both. Colivelin is a different molecule: HNG17 with a C8R change, joined to the activity-dependent neurotrophic factor peptide ADNF (PMID 16267233); it is not covered by this record. Natural level studies are a large part of the literature. Circulating HN fell with age in humans and mice in one report (PMID 19623253). Plasma HN rose in old age and was highest in centenarians in a cohort of 693 people aged 21 to 113 (PMID 29955888). Circulating HN was higher in children of centenarians than in matched controls, and lower in Alzheimer disease and MELAS, in another report (PMID 32575074). These findings conflict on the age trend. No human trial that gives HN or HNG to a person was found. ClinicalTrials.gov lists 7 studies that mention humanin as of 2026-10-13. All of them measure HN levels or use HN as an outcome, and none tests it as a treatment. HN is not in the FDA 503A or 503B compounding lists or the FDA category 2 page, has no Drugs@FDA record, and has no entry in the EMA medicines or orphan designation data files. WADA does not name it in the 2026 or the 2027 List. The S0 class covers any pharmacological substance that no other section addresses and that has no current approval by a governmental regulatory health authority for human therapeutic use, so S0 applies by its wording (this is my reading of the List text, not a WADA statement).
