Thymosin alpha-1 is the N-acetylated 28-residue N-terminal peptide of human prothymosin alpha (UniProt P06454, residues 2-29). MeSH describes it as a thymus peptide found in thymosin fraction 5 and now made by synthesis (INN thymalfasin). Mouse and cell studies report dendritic-cell activation through Toll-like receptor signalling (TLR9/MyD88); ChEMBL records the mechanism as unknown. Zadaxin (SciClone) was approved for chronic hepatitis B in Singapore, China and the Philippines by 1997; it has US and EU orphan designations but no FDA or EMA approval.

54 safety results for Thymosin alpha-1.
In compensated HBV cirrhosis, both entecavir plus thymosin α1 and entecavir alone were reported as well tolerated.
Randomized controlled trialn = 690
In hepatitis C patients, the incidence of adverse events did not differ significantly between thymosin alpha‑1 and placebo groups.
Randomized controlled trialn = 552
In patients with predicted severe acute necrotising pancreatitis, new-onset organ failure did not differ between thymosin α1 and placebo.
Randomized controlled trialn = 508
In ICU patients with severe sepsis, no serious drug-related adverse event was recorded with thymosin alpha‑1.
Randomized controlled trialn = 361
In Japanese adults with chronic hepatitis B given thymosin alpha‑1 0.8 or 1.6 mg, all adverse drug reactions were mild and most involved fluctuation of liver enzymes.
Randomized controlled trialn = 316
In dialysis patients, 3 deaths occurred in the thymalfasin group compared with 7 in the control group; no statistical test is given.
Randomized controlled trialn = 194
In patients with tuberculosis and diabetes, adding thymosin alpha‑1 to chemotherapy did not change the incidence of adverse reactions.
Randomized controlled trialn = 120
In chronic hepatitis B, no significant side effects were observed with Talpha.
Randomized controlled trialn = 98
