Vasoactive intestinal peptide (VIP) is a linear peptide amide of 28 residues (HSDAVFTDNYTRLRKQMAVKKYLNSILN-NH2) with a free N-terminus and no disulfide bond. It is cut from the 170-residue human precursor (UniProt P01282, VIP_HUMAN) at positions 125-152, and Asn152 is amidated. The same 28 residues are in the UniProt entries for mouse, rat, cattle, pig and other mammals. Aviptadil is the INN of synthetic VIP with this sequence. VIP binds the class B receptors VPAC1 and VPAC2 with high affinity. PAC1 is selective for PACAP. Most of the literature is about the body's own VIP (gut, nerves, brain, immune cells). As a drug, aviptadil is authorised only inside Invicorp (aviptadil 25 micrograms with phentolamine 2 mg, injected into the penis) for erectile dysfunction, for example in Sweden since 2015-05-07. FDA has not approved it and EMA has not centrally authorised it. The largest trial of aviptadil alone, TESICO (phase 3, NIAID, COVID-19 respiratory failure, 471 people randomised), found no benefit at day 90 (odds ratio 1.11, 95% CI 0.80-1.55) and stopped for futility in 2022. FDA lists 'Vasoactive Intestinal Peptide' in 503A compounding category 1 (under evaluation).

| US | FDA | No Drugs@FDA record: openFDA drugsfda searches for aviptadil, 'vasoactive intestinal' and the brand Zyesami returned NOT_FOUND on 2026-10-09. The FDA page on emergency use authorisations for drugs (content current as of 2026-07-01) does not list aviptadil. NRx Pharmaceuticals reported that FDA declined an EUA for Zyesami in a patient sub-group (trade press, 2022-07-04; no FDA document found). Four FDA orphan designations, all 'Designated' and 'Not FDA Approved for Orphan Indication': vasoactive intestinal polypeptide, 1993-06-23, acute esophageal food impaction (Research Triangle Pharmaceuticals); vasoactive intestinal peptide, 2001-03-09, acute respiratory distress syndrome (mondoBIOTECH); aviptadil, 2005-02-22, pulmonary arterial hypertension (Mondobiotech Laboratories AG); aviptadil, 2020-07-23, sarcoidosis (Relief Therapeutics International SA). The FDA orphan-database search page returned HTTP 503, so only these four (the codes held by GSRS) were checked. | 9 Oct 2026 | fda.gov (opens in a new tab) |

29 safety results for Vasoactive intestinal peptide (VIP, aviptadil).
In adults with COVID-19 acute hypoxaemic respiratory failure, the combined primary safety outcome up to day 5 was not significantly different between intravenous aviptadil (63%) and placebo (56%).
Randomized controlled trialn = 461
In patients with COVID-19 respiratory failure, the authors report no drug-related serious adverse events with IV aviptadil.
Randomized controlled trialn = 196
In hospitalized adults with COVID-19, no participant dropped out of the trial because of side effects of inhaled aviptadil.
Randomized controlled trialn = 80
In people with migraine without aura, intravenous VIP infusion did not trigger a migraine attack in any participant.
Randomized controlled trialn = 12
In people with migraine without aura, VIP infusion caused a mild immediate headache that was greater than with placebo.
Randomized controlled trialn = 12
In people with migraine without aura, delayed headache 3 to 11 hours after infusion did not differ significantly between VIP and placebo.
Randomized controlled trialn = 12
In healthy volunteers, headache was very mild (maximum score 2); five participants had headache during VIP infusion and one during placebo.
Randomized controlled trialn = 12
In healthy volunteers, mean arterial blood pressure did not differ between VIP and placebo days.
Randomized controlled trialn = 12
In hamsters and in mice, VIP treatment had no effect on body weight.
