NAD+ (nadide is the INN, USAN and JAN name; also coenzyme I, DPN and codehydrogenase I) is a dinucleotide: a nicotinamide nucleotide joined by a pyrophosphate bridge to an adenosine nucleotide. The formula is C21H27N7O14P2 (663.4). It is also a substrate that enzymes use up: UniProt lists NAD(+) in the catalytic reactions of PARP1 (poly-ADP-ribosylation), SIRT1 (protein deacetylation), CD38 (cyclic ADP-ribose and ADP-ribose formation) and SARM1 (NAD(+) hydrolysis). No approved NAD+ drug product was found in the US. Drugs@FDA (openFDA) has no match for nadide, and the DailyMed search for 'nadide' returns homeopathic combination products and one oral dietary supplement label (Viatrexx-NAD Plus, 500 mg powder, last revised 2019). FDA lists NAD in 503A category 1 (under evaluation) and beta-NAD in 503B category 1, and the disodium salt trihydrate in category 3 of both lists. FDA warned compounders on 2024-10-30 that food-grade NAD+ is not suitable for sterile IV products and said it had adverse event reports after NAD+ injectable drugs (severe chills, shaking, vomiting, fatigue) that fit high endotoxin levels. An NAD+ for injection product from GenoGenix LLC was recalled in a Class I recall (elevated endotoxin, recall initiated 2025-07-30, reported 2025-10-15, status ongoing). In China, coenzyme I for injection (Enaidi, now sold by Knature Biopharmaceutical, Hefei, a company founded in 2017) was first approved in 2002 according to Chinese news reports about the company's Hong Kong listing filing; the reported label uses are adjuvant treatment of coronary heart disease, myocarditis and leukopenia, by intramuscular injection. The NMPA record itself was not read. Chinese regulator registrations (CDE) list phase 1 and phase 2 trials of coenzyme I for injection in heart failure and ovarian function decline. WADA does not name NAD+ on the 2026 or 2027 Prohibited List. Rule M2.2 limits IV infusions and injections to 100 mL per 12 hours outside hospital treatment, surgery and diagnostic work. Direct NAD+ studies in people are few and small. Examples: a 2019 pilot of a 6 hour IV infusion in people (PMID 31572171, title read only), a 2026 real-world comparison of 500 mg NAD+ IV with NR IV for 4 days that reported moderate to severe gastrointestinal symptoms, faster heart rate and chest pressure with NAD+ (Frontiers in Aging), and a 2026 randomized trial of IV NAD+ 10 mg per day for 7 days in heart failure (PMID 40954388, not read beyond the abstract). Science-Based Medicine reports a February 2026 systematic review that found no eligible controlled outcome trials of IV, IM or SC NAD itself for anti-aging or wellness. The same commentary reports the review authors' argument that NAD+ is a large polar molecule not expected to cross cell membranes intact. Oral NMN and NR (precursors) are different compounds and are not described here.

| US | FDA | No Drugs@FDA record: openFDA drugsfda search of the active ingredient NADIDE returned no match (2026-10-13). DailyMed SPL search for drug name 'nadide' returned 7 labels: six combination products (GUNA-MATRIX, GUNA-ARTHRO and four ZEEL labels, which the search matched on the name) and Viatrexx-NAD Plus powder (version 2019-08-26), which DailyMed marks as a dietary supplement for oral use ('Dissolve in water and take orally'). The search for 'nicotinamide adenine dinucleotide' returned 0. GSRS gives NAD a DSLD code (Dietary Supplement Label Database, 'Number of products: 105') and an NCI concept 'Dietary Supplement'; a DSLD full-text search for the phrase on 2026-10-13 returned 271 labels. FDA safety reminder (content current 2024-10-30): FDA is aware of compounders using food-grade NAD+ sold by repackagers to make intravenous products, which carries a high risk of microbial and endotoxin contamination, and FDA has received adverse event reports after NAD+ injectable drugs, including severe chills, shaking, vomiting and fatigue, with some needing medical treatment, consistent with excessive endotoxin. openFDA enforcement reports list 14 records with 'nicotinamide adenine' in the product description (2017 to 2026; at least one is an NADH product); 13 are Class II (mostly lack of sterility assurance, one for sub-potency and others for reconstitution time or labelling) and one is Class I: D-0094-2026, GenoGenix LLC, NAD+ for injection 100 mg/mL and 200 mg/mL multi-dose vials, microbial contamination (elevated endotoxin levels), recall initiated 2025-07-30, report date 2025-10-15, status ongoing, nationwide. The same firm had a separate Class II recall of NAD+ 50 mg/mL and 100 mg/mL vials (D-0065-2026). Science-Based Medicine (2026) says there is no FDA-approved NAD+ IV product from a traditional drug maker, so any infusion is a compounded product. | 13 Oct 2026 | fda.gov (opens in a new tab) |

| China | National register | Approved in China Approved, according to Chinese news reports. Coenzyme I for injection ('Enaidi' in Chinese), Knature Biopharmaceutical (Hefei, Anhui), first approved in China in 2002 (Jiemian / Daily Economic News, from the company's Hong Kong listing filing). Reported label uses: adjuvant treatment of coronary heart disease, myocarditis and leukopenia (Jiemian, which cites 'public information'; Guangming Online 2026-09-06, credited there to CCTV News). The Guangming Online text itself states that the legal route is intramuscular, and it quotes a surgeon who says one dose is 5 mg (an expert statement, not label text). It also says no anti-aging indication exists. Jiemian says it is the only approved drug that directly contains NAD+. Jiemian also reports sales of the product (about 155 million yuan in the first nine months of 2025, 65.2 percent of the company's revenue) and says the company was founded in 2017. The reports do not name who held the approval in 2002. A ClinicalTrials.gov protocol (NCT07839130) names Knature Biopharmaceutical Co., Ltd. and calls the product a marketed product with national drug approval no. H41024721 and 5 mg per vial. The NMPA drug database and the label text itself were not read, so the approval number, the label dose and the current status are not confirmed. Chinese regulator (CDE) trial registrations for coenzyme I for injection: CTR20231677 (phase 1 in healthy people, completed), CTR20252331 (phase 2, heart failure after STEMI, recruiting), CTR20261623 (phase 2, ovarian function decline, 50, 150 and 300 mg, not yet recruiting). | China (Guangming Online, credited to CCTV News, 2026-09-06) | Note | Report of a woman in a Beijing cosmetic clinic who had an IV drip of an NAD+ 'anti-aging' coenzyme I injection. About 20 minutes into the infusion she had palpitations, chest tightness and a burning feeling in the blood vessels of both arms, then days of insomnia and headache; a hospital linked the symptoms to too much NAD+ given. The article says China has never approved any medicine with an anti-aging indication. | 13 Oct 2026 | m.gmw.cn (opens in a new tab) |
| Japan | Note | KEGG DRUG D00002 (Nadide, JAN/USAN/INN) is a drug entry with ATC code A11HA01 as printed there, efficacy 'Coenzyme' and a comment 'Antagonist (to alcohol and narcotics)'. It lists no Japanese product. The Japanese product group linked in that entry (D00036) is for nicotinamide, a different compound. No Japanese NAD+ product was found. PMDA was not searched. | 13 Oct 2026 | rest.kegg.jp (opens in a new tab) |
