Orforglipron (LY3502970, OWL833) is a non-peptide small molecule, C48H48F2N10O5, about 883 Da, that binds and activates the human GLP-1 receptor. Chugai Pharmaceutical discovered it and Lilly licensed it in 2018 (Lilly release of 2026-09-30). The 2020 PNAS paper (PMID 33177239) describes it as a partial agonist biased toward G protein activation over beta-arrestin, with a cryo-EM structure (PDB 6XOX) that shows a binding pocket in the upper helical bundle of the receptor. The US label says it is not pharmacologically active in rats and mice and caused no rodent tumors. It is metabolised mainly by CYP3A4, reaches peak concentration 4 to 8 hours after a dose, and has an elimination half-life of about 29 to 49 hours; absolute bioavailability was 77 percent after a 0.8 mg dose (label sections 12.1 and 12.3). FDA approved the calcium salt as Foundayo tablets on 2026-04-01 under NDA 220934 (Type 1 new molecular entity), under the Commissioner's National Priority Voucher pilot, 50 days after filing (FDA bulletin of 2026-04-01). The US indication is, with a reduced-calorie diet and more physical activity, to reduce excess body weight and keep it off in adults with obesity, or with overweight and at least one weight-related condition. The label dose is 0.8 mg once daily, stepped up after at least 30 days at each step through 2.5 mg and 5.5 mg to 9, 14.5 or 17.2 mg, with a maximum of 17.2 mg once daily, with or without food. The label has a boxed warning about thyroid C-cell tumors. The label says its trial data come from an investigational formulation and shows them as equivalent label dosages; published trials used capsule doses of 6, 12 and 36 mg (PMID 40960239). Lilly reported a US type 2 diabetes submission in the second quarter of 2026. As of 2026-10-13 no FDA decision on it was found, and the current US label lists weight management only. The UK MHRA authorised it on 2026-08-10 for weight management and for type 2 diabetes. The EMA lists an application for obesity and type 2 diabetes as under evaluation (data extracted 2026-10-05). The UAE Emirates Drug Establishment announced its approval on 2026-04-03 (press release carried by Zawya and TradeArabia), and the Saudi Food and Drug Authority approved the registration on 2026-08-02 (SFDA statement quoted by Khaleej Times). WADA does not name it on the 2026 or 2027 List. Before approval, research-chemical sellers sold products labelled orforglipron; Lilly said in October 2025 that no one could legally sell it for human use.

Top outcomes
The 8 outcome groups with the most study results, by evidence class.
Data version 308560. Bar length is the number of results, not the size of an effect.
Evidence matrix
Study results by outcome and evidence class. Arrows show how the measured outcome moved, not whether the change is good.
